Windows XP

  1. Click on the Start button, then click "Run..."

     
  2. Type "cmd" into the Open field and click "OK"

     
  3. In the black window that appears, type in "ipconfig/all" and press enter.
     
  4. Scroll up through the information and look for the heading "Ethernet adapter Local Area Connection"
     
  5. Your computer's MAC address will be displayed next to "Physical Address"

     
  6. Record this number and input it into the IP Request Form.
